This Privacy Policy outlines how ohipHCV.ca collect data from users and how it protects and utilizes the information. It also addresses the scope users have as to the collection, correction, transmission, retention and use of the information. It is the policy of ohipHCV.ca to keep any information gathered through the use of these tools as private. As such, user information is not disclosed or shared to unauthorized third parties except as allowed by Canadian law.
PERSONAL IDENTIFIABLE INFORMATION.
A user always has the opportunity to contact ohipHCV.ca to inquire about any of our services. During the process of addressing these inquiries or the process of registering for our services, users may be asked to submit Personal Identifiable Information. This includes such information as name, title, organization name, phone number, e-mail or address and information about your job function. ohipHCV.ca maintains security and privacy policies and procedures to ensure the integrity of the data in our care. We will not sell, disclose or share with third parties or any other unauthorized person except in the manner that is provided by law.
ohipHCV.ca maintains the right to inform our users about any change that may affect information collected or stored. We may be required to comply with a court order or governmental regulatory requirement or disclose information to our lawyers in connection to legal proceedings or in line with business practices that are to ensure quality control and maintain a stable operating system.
When conatced by a user to troubleshoot a problem, ohipHCV.ca reserves the right to use the user's info in the process of troubleshooting in order to solve a problem or identify cause. This will not involve disclosure or sharing of the info unless explicitly approved in writing by the user.
ohipHCV.ca reserves the right to use contact information regarding any aspect of their account or the corresponding services. Users will have the option to opt out of the optional communications (marketing, events) by writing to ohipHCV.ca, while the mandatory communications (security updates, product announcements/revisions) will go out to all users. Please be aware that this privacy policy and any choices you make will not necessarily apply in some cases if contact with the customer is an intrinsic part of the product or service that we provide.
ohipHCV.ca is not anticipating any changes in corporate status. Should we be subject to a takeover bid, merger, or divestiture of assets, ohipHCV.ca may disclose, transfer or sell to the surviving or acquiring entity respectively. Personally identifiable information collected online remains subject to applicable legislation and corporate policy.

OBLIGATIONS TO THE MINISTRY We are responsible for providing to the Ministry of Health and Long Term Care, on demand, audit information describing specific transaction or transaction sequences in the fashion requested by the Ministry.
PATIENT HEALTH INFORMATION
All patient health information is encrypted and stored on professionally administered secure servers as required by provincial and federal law. Any patient health information collected through ohipHCV.ca services are not combined with other user data. Patient information will not be shared or communicated with third-party companies.
